Hazard report form

Please note that Bikewest coordinates the development of the Perth and Regional Bicycle Networks but does not own the infrastructure once it is built. Bikewest is providing this form to help people report cycling hazards to the appropriate authority in the Perth metropolitan area.

For hazards located in regional areas of Western Australia please copy the form and contact the relevant local authority directly or call the Main Roads WA hazard reporting line on 138 138 which is open 24 hours a day.

For hazards within the Perth metropolitan area, including Mandurah, please complete and return this form to the local government responsible for attending to the hazard.

To assist you in determining which metropolitan local government is responsible, two maps have been provided. The PSP Map (pdf 2.6MB) shows which government organisation is responsible for each section and a Local Government Boundaries map (pdf 3.1MB) for all other areas have been provided.  You may find it helpful to check a street map or a Google map to find out the street names and help identify the location of the hazard.

Please note that traffic lights, major principal shared paths alongside freeways, major highways and railway tracks, are usually the responsibility of Main Roads WA. In that case a report should be sent to Main Roads WA or call their hazard reporting hotline, open 24 hours a day, on 138 138 .

Shared paths in railway precincts are the responsibility of the Public Transport Authority which should be notified of hazards on www.pta.wa.gov.au.

If you are involved in a crash when riding a bike it is mandatory that you report this to the Police if someone else sustains bodily injury, there is $1,000 total damage or the owner of damaged property is not present. This can be done via 24 hours, 7 days a week online crash reporting web site www.crashreport.com.au. Special insurance coverage for cyclists can be obtained in WA through cycling groups such as Cycling WA, Bicycling Western Australia and the Bicycle Transport Alliance and some domestic contents-insurance policies.
