Proof of age card

A proof of age card provides anyone aged 18 years or over with a recognised form of personal identity, primarily for use in accessing licensed premises in Western Australia. Find out how to obtain a proof of age card, or order a replacement.

How to apply for a proof of age card

Step 1: Complete the proof of age card application form

You can download the form below or pick one up from a Driver and Vehicle Services Centre or regional Agent. If you would like a copy posted to you please contact us.

Step 2: Attend a Driver and Vehicle Services Centre or regional Agent

When applying for a proof of age card, you must attend a Driver and Vehicle Services Centre or regional Agent in person. You will need to:

  • Provide one primary and one secondary proof of identification documents - one of which must contain your signature (accepted proof of identification documents are provided in the application form below)
  • Provide the completed application form
  • Pay a fee (refer to schedule of fees)
  • Have your photograph taken. Please note that supplied photographs will not be accepted. Your photograph will be taken by the Driver and Vehicle Services Centre or regional Agent.

Step 3: Application will be processed and card manufactured

If you need your proof of age card for a special occasion, please remember to allow sufficient time for the manufacture and postage of the card after your application is lodged.

The process normally takes five to eight working days.   

Important notes

  • Completed application forms must be presented in person
  • You must be aged 18 years or over to apply for a proof of age card
  • You must provide one primary and one secondary proof of identification documents - one of which must contain your signature (accepted proof of identification documents are provided in the application form below)
  • A student ID card is not an acceptable form of identification
  • You must supply a Western Australian residential address
  • It is an offence to give false or misleading information. Penalties will be enforced under the Liquor Control Act 1988 and Liquor Control Regulations 1989Fraudulent applications will be referred to the WA Police
  • There is no exemption from the photograph requirement.  All proof of age cards issued must display a photograph of the card holder

How to apply for a replacement card

If you need to apply for a replacement  proof of age card you must attend a Driver and Vehicle Services Centre or regional Agent in person.

Remember to provide one primary and one secondary proof of identification documents - one of which must contain your signature (accepted proof of identification documents are provided in the application form above) .

Please note that Driver and Vehicle Services Centre or regional Agents do not have access to previously captured images to view or initiate production of a replacement proof of age card. A new image will need to be captured in order to produce a new proof of age card.

What is a proof of age card?

The proof of age card is a personal identity card which is available to anyone aged 18 years or over.

The card fits easily into your purse or wallet and shows only those details considered necessary for identification, including your:

  • Name
  • Date of birth
  • Signature
  • Photograph.

For security reasons, your card does not show your address.

The proof of age card is designed primarily for accessing licensed premises. Please note that there is no obligation for institutions that require a person to prove their age and identity to accept the proof of age card.
