Good behaviour period election form

This form should only be used if you have been served with a Excessive Demerit Point Notice and you wish to elect to take the Double or Nothing option and drive on the Good Behaviour Period.

To be eligible to make this election you must:

  • Hold a valid Western Australian driver's licence,

  • Must not be a provisional driver

  • Submit your election information to us within 21 days of being served your Excessive Demerit Point Notice and

  • Supply all the information requested in the form below marked with an asterisk (*).

Please note:

  • Elections can only be made by the person who has been served the Excessive Demerit Point Notice

  • If your licence is currently disqualified due to Fines Enforcement or Court Conviction the Good Behaviour Period will not commence until these disqualifications have been completed.

  • Any election received later than 21 days from when the Excessive Demerit Point Notice has been served will not be accepted. The disqualification period will commence 28 days after receiving the Excessive Demerit Point Notice.

  • The Good Behaviour Period will commence when your disqualification was due to begin

By making this application you understand that if you:

  • accrue more than 1 demerit point for offences committed during the Good Behaviour Period or
  • commit an offence during the Good Behaviour Period that results in disqualification

You will breach the Good Behaviour Period. You will be disqualified for double the time of your original demerit point disqualification.
