Publications and Forms

There are a number of publications and forms to assist in managing the Walking School Bus program.

• Brochure  (pdf 1.7MB) – aimed at parents to volunteer for a Walking School Bus
  
  
• Online Route Record Form  – to be filled out by each Volunteer Coordinator after Terms 1, 2 and 3.  The form contains information including the number of children and volunteers walking; number of trips the bus made and the number of first time walkers.
  
  
• Survey form (pdf 80kb) – for distribution to schools to gauge parent participation to join an existing Walking School Bus, or generate a new route to the school.
  
  
• Parent Consent form  (pdf 40kb) – signing this form allows their child to catch the Walking School Bus to their primary school
