Next steps for Samir
Step 1: Understanding the Requirements
As a non-Australian resident, Samir cannot use overseas documents to create the link for his name change. Instead, he must present the FoI decision letter (not application letter) from DHA confirming his approved name change.
Step 2: Obtain a DHA FoI Decision Letter
Before changing his name with DTMI, Samir completes a Request for Amendment or Annotation to Personal Records (424C) Form obtained via the Department of Home Affairs (DHA) website.
Samir notes on the form he is a WA Department of Transport and Major Infrastructure customer that requires a FOI letter with a screenshot of the visa database showing his name details. You may first need to change your name with DHA if the name in their database is not what you wish to be known by.
Step 3: Preparing the Required Documents
Primary and Secondary Proof of Identity (POI)
To meet DTMI requirements, Samir needs to provide Primary and Secondary proof of identification documents. He notes the documents must include a signature. After reading the list of documents Samir chooses to bring in:
- His current WA driver’s licence (Primary POI)
- His bank card with signature (Secondary POI)
Form C4 – Change of Personal Details
Samir completes a Change of Personal Details - Form C4 [PDF 261kb] to formally request the name change.
DHA FoI Decision Letter
This official letter confirms that the Department of Home Affairs has approved Samir’s name change request to his new name.
Step 4: Visiting a DTMI service centre
Samir visits a DTMI service centre with all his documents. He submits:
- His completed Form C4 Change of Personal Details
- His Primary and Secondary Proof of Identity documents
- His DHA FoI decision letter
He must pay $33.30 for a replacement driver’s licence card.
