Change my name

If you've changed your name, you need to let us know.

If you’ve changed your name after marriage, the Registry of Births, Deaths and Marriages has information on how to register your name change.

What you need to do

You can update your name in person at DTMI service centre or regional agent

You'll need to bring: 

  • a completed Change of Personal Details (Form C4)
  • primary and secondary proof of identity of which: 
    • 1 is primary and 
    • 1 is secondary
  • originals of any documentation relating to your name change e.g. marriage, change of name or birth certificate issued by Registry of Births, Deaths and Marriages or Freedom of Information (FOI) decision letter.

Please note:

  • your identity and change of name documents should show a clear link between your names
  • at least 1 of the documents should contain your signature
  • commemorative marriage certificates will not be accepted.

Replacement card or document fees

There is no fee payable to change your name, organisation name, address or gender.

However, you will need to pay a fee if you request a replacement driver's licence card, WA photo card, vehicle licence document, or learner’s permit card.
